Your first visit in our office will include a thorough evaluation by Dr. Smith and may require x-rays or other diagnostic tests. You will be asked to complete a patient information form and an in-depth foot and ankle history questionnaire prior to being seen for your first visit. Please download these forms from our web site under Forms. The forms allow the doctor to efficiently review a significant amount of information. It is helpful for you to obtain and bring to the office any x-rays, bone scans, MRI scans, CT scans, or other information pertinent to your previous care. It may be appropriate for our staff to order x-rays of your feet and/or ankles to be done at a nearby facility prior to your visit (often on the day of your evaluation).

At the end of your visit, you may be asked for permission to take your picture for our electronic medical record. Photo documentation of your foot and ankle problem may also be taken for the record. Your picture is for our records only. We may ask to use it with your permission, if you give a testimonial regarding your care at BOFAC. Anatomical photos are sometimes used for conferences or medical education, but there is no name or facial identification with the foot/ankle photograph. PLEASE NOTIFY US IF YOU DO NOT WISH TO HAVE YOUR FOOT/ANKLE PHOTOGRAPH USED FOR THESE EDUCATIONAL PURPOSES.

It is our policy to send a report of the examination finding to your referring physician as well as to your primary care physician if you would like. All information given to us is strictly confidential and will not be released to any parties other than your physicians without your consent.
